An elder male patient with dyspeptic symptoms and progressive jaundice with weight loss over 5kg. in 2 months, CT showed large inhomogenous with low enhancing mass size around 5x8cm at right liver close to surface and proximal bile duct with hilar to both IHD dilatation, more dilatation on left IHD than right IHD, collapsed distal CBD. ERCP showed subhilar tight stricture about 18-20mm long with upstream dilatation from 1+cm subhilar area till hilar and both IHD, after brushing cytology-intraductal biopsy done, a SEMS diameter of 10mm, 100mm long was selectively put from left IHD to duodenum for drainage. An oval tip of SEMS was transiently stuck at narrowest part of stricture after SEMS deployment, traction with move duodenoscope tip around the ampulla for a while can be help in extraction the oval tip device out of ampulla without need for additional dilatation of SEMS.
