Xanthogranulomatous Pyelonephritus is a chronic, infective condition of kidney which leads to progressive destruction of renal parenchyma. Pathogenesis is by interplay of three factors: starting with an obstructive calculus, obstruction and subsequent infection. Histopathologically, they exhibit lipid laden macrophages in a background of inflammatory cells. Differentiating a kidney affected with xanthogranulomatous Pyelonephritus from clear cell renal cell carcinoma can be difficult, as ccRCC also show lipid laden macrophages, leading to unnecessary radical nephrectomies. It is usually an unilateral condition affecting diabetic females. Proteus is the most common organism cultured, followed by E. Coli CT scan helps us to diagnose the condition.
