Under ultrasound guidance, the exact nature of the content can be studied. The cystic content can be suctioned (aspirated) out therefore causing the cyst to be smaller (i.e decompressed) though however this depends largely on the viscosity of the fluid content. If watery enough to be able to be sucked through needle, then decompression can be achieved. Treatment options for symptomatic benign cystic or predominately cystic thyroid nodules include needle aspiration, minimally invasive techniques like percutaneous ethanol (alcohol) injection (PEI) besides surgical resection. A prior biopsy or cytology study is mandatory to prove the benign nature of the nodule. With PEI under ultrasound guidance, generally should be considered, the ethanol results in permanent tissue damage with subsequent necrosis, fibrosis and thrombosis of cyst wall blood vessels. IMPORTANT CONCEPTS: Not all thyroid nodules (whether cystic or solid) need any form of surgery. Some have no symptoms and can be placed under active surveillance ultrasound scans. -Some nodules would need biopsy to know if it is cancerous or not. -Ultrasound-guided biopsies would surely have much greater accuracy (close to 95%) as compared to non-ultrasound-guided biopies. -Not all thyroid nodules need removal surgery in the form of “lobectomy”, also known as “hemithyroidectomy” (whether remote access/endoscopic/robotic). -All thyroid cysts can have interventional procedures (Ultrasound-guided drainage with ablative techniques) done without any removal surgery. -Ultrasound-guided cyst drainage procedures would surely have much better outcome as compared to non-ultrasound-guided procedures. -All non-cancerous nodules and even early cancer have a minimally-invasive ablative treatment option before even considering removal surgery -Total thyroid removal will have its roles especially in very enlarged necks and cancer or perhaps some cases of stubborn hyperthyroidism but now lesser indications than before Please do drop by for your 2nd 3rd etc for our opinion (we even have a 7th one, by the way).
