Hot Axios EUS Guided Cystogastrostomy | Pseudocyst/WON | CGS

EUS-Guided Targeting : A using endoscopic ultrasound probe, the pseudocyst/WON was localized. The cyst was visualized with clear margins and confirmed to be in close apposition to the gastric wall. No major vascular structures were noted in the needle path on Doppler evaluation. The pseudocyst/WON measured approximately 65x90mm. Cyst wall to stomach wall distance- 11.53mm. Cystogastrostomy and LAMS Placement: A Boston Scientific 10 mm × 20 mm cautery-enhanced LAMS was used for direct access to the pseudocyst/WON without the need for a pre-puncture needle. The stent was deployed successfully under real-time EUS guidance with the cautery-enabled delivery system. Deployment was smooth, with the distal flange opening within the pseudocyst/WON cavity and the proximal flange within the gastric lumen. Post-deployment, adequate placement of the stent was confirmed endoscopically and via ultrasound. Immediate drainage of infected fluid and necrotic material was observed through the stent into the stomach. No bleeding or complications were noted during the procedure.
