Hi everyone, my name is Matt Aquilina with Nexus Baseball and this is Safe Baseball, where I quickly cover your questions about injury prevention when training for baseball. After going over the Tommy John Epidemic in the last episode I now want to cover what Tommy John surgery is and what it entails. The pioneering surgery was created by Dr. Frank Jobe and first performed on pitcher Tommy John in 1974. The procedure was thought to have an extremely small chance of success, but John beat the odds and later returned to pitching, having a long career and pitching more years after his surgery than before it. The ulnar collateral ligament, abbreviated as the UCL, is a triangular ligament made up of three parts that connect the bone of the upper arm, the humerus, with one of the two bones in the forearm, the ulna. The ligaments of the human body are connective tissues that provide the framework that holds the skeleton together under the pressures of the muscles that create human motion. The primary role of the UCL is the stability of the arm and to ensure the lower arm does not separate from the upper arm at rest and during motion. When stresses on the elbow joint exceed the body’s ability to use its muscles to protect this small ligament, it can fray or tear, causing pain and the inability to hold the elbow joint together. Known formally as UCL reconstruction surgery, Tommy John surgery is a procedure to repair a torn UCL ligament by using a tendon to replace it. The tendon is harvested from either the palmaris longus in the forearm, a hamstring tendon, or a big toe extensor tendon. An incision is made outside of the elbow and the damaged UCL is removed. Holes are drilled into the humerus and ulna bones and the new tendon fed through these holes to bind the two bones together. There are many different surgeons that now perform the procedure across North America. The surgery has a high rate of initial success of 80% and a return to play time of approximately a year for most players. Although a successful surgery from an orthopedics perspective, the surgery that was one undertaken a few times a year has exploded into thousands of procedures a year, particularly in children, forcing a lot of pain and a tough rehab, as well as generally shortened careers.
