Internal Thoracic Artery (ITA) - Origin: Arises from the subclavian artery. Course: Descends vertically along the inner surface of the thoracic wall, just lateral to the sternum, within the thoracic cavity. Branches: Pericardiophrenic Artery: Accompanies the phrenic nerve, supplying the pericardium and diaphragm. Anterior Intercostal Arteries: Supply the first six intercostal spaces. Musculophrenic Artery: Supplies the lower intercostal spaces and diaphragm. Superior Epigastric Artery: Continues downward into the anterior abdominal wall. Internal Thoracic Vein (ITV) - Course: Accompanies the internal thoracic artery. Tributaries: Receives blood from the anterior intercostal veins, superior epigastric vein, and musculophrenic vein. Drainage: Drains into the brachiocephalic vein. Clinical Significance - Coronary Artery Bypass Grafting (CABG): The internal thoracic artery, especially the left internal thoracic artery (LITA), is commonly used in coronary artery bypass surgery due to its superior long-term patency rates.
