Although the scar was small, it was located near the eyebrow — a highly visible area — which made the patient very conscious. Facial scars, even tiny ones, can affect confidence because they are always noticeable. Many people try creams, gels, or home remedies for years without improvement. This case helps explain why some scars don’t respond to creams or lasers and why proper evaluation is essential. Understanding Scars Is Complex Scar treatment is one of the most challenging areas in plastic surgery. There is no single treatment that works for all scars. Management depends on: • Size of the scar • Location on the face • Whether the scar is raised, flat, or depressed • Skin color and scar color • Age of the scar • Whether it is stuck to deeper tissue • Presence of hair-bearing skin nearby In this case, the scar was: ✔ A depressed (pitted) scar ✔ Caused by chickenpox infection ✔ Located near the eyebrow ✔ Adherent to deeper tissue Chickenpox scars form because infection damages deeper layers of skin, creating a small “pit” that sticks downwards. Why Creams and Silicone Gels Don’t Work Many patients use: • Scar creams • Silicone gels • Silicone tapes • Home remedies These may help fresh scars, but once a scar becomes mature (older than 1–2 years), creams cannot fill a depression in the skin. What About Lasers? Lasers have specific roles: • Q-switch laser – improves dark scar color • CO2 laser – smoothens raised scars • Injections – help hypertrophic or keloid scars But in deep, color-matched, depressed scars, lasers alone cannot correct the indentation. Other Options Considered • Subcision + Fillers – temporary improvement (lasts ~1 year) • Fat grafting – better for multiple scars, not ideal for a single small scar Since the patient wanted a permanent solution, these options were not ideal. The Chosen Treatment: Scar Revision Surgery We performed scar revision surgery, which involves: • Precisely excising the old scar • Releasing underlying adhesions • Fine, layered suturing for minimal tension • Aligning skin edges carefully The procedure: ✔ Done under local anesthesia ✔ Took around 20 minutes ✔ Sutures removed after 7 days Post-Surgery Care Matters Most Proper aftercare is crucial: • Use steri-strips to prevent scar stretching • Silicone gel or tape after healing • Possible laser sessions later for color blending Scars take months to mature, and gradual improvement continues.
