People with chronic conditions, such as heart disease and stroke, and especially those with multiple chronic conditions, receive care from numerous providers in various settings and regularly juggle multiple prescription drugs, making care coordination key. Evidence suggests that use of health information technology can help manage chronic diseases, such as heart disease and stroke, more effectively by facilitating communication and coordination of care between providers and patients across settings and improving patient safety by reducing duplicative tests, procedures, and costly medical errors. One of the contributing factors to cardiovascular mortality in the U.S. is the inability to monitor heart rhythms of patients with heart disease until they arrive in the emergency room. Thus, heart disease management is effective and efficient when it involves big data analytics - the examination of large amounts of data to uncover hidden patterns and unknown correlations - that can be used to monitor, predict, prevent, manage, and treat chronic diseases, such as heart disease and stroke. Here, we propose creating a new health information technology, a wearable device software application, called SciRyng, that can monitor and respond to the care of people with chronic diseases, such as heart disease and stroke, quicker and more efficiently than current methods in the healthcare market using artificial intelligence-driven predictive analytics.
