Silfverskiold Test for Ankle Contracture

The Silfverskiöld Test was described by Dr. Nils Silfverskiöld in 1924 (Silfverskiöld, N., Reduction of the Uncrossed Two-Joints of teh Leg to One-Joint Muscles in Spastic Conditions. Aca Chirurgica Scandinavia, 1924). In this article he discussed how muscles such as the hamstrings and gastrocnemius cross two joints and how their effect on one joint affects the other. He talks about how people with spasticity in the leg will affect things such as walking so this test is a means to determine if someone has an Achilles contraction or just tightness in the gastrocnemius. How to perform the test -Have the patient seated, prone or supine -With the knee extended, dorsiflex the ankle -Then bend the patient’s knee to 90 degrees and dorsiflex the ankle -Compare the amount of dorsiflexion in both positions. If the dorsiflexion is more with the knee flexed, the gastrocnemious is tight. If the dorsiflexion is the same with the knee flexed and extended, it could be an Achilles contracture or soleus tightness There is not much research on this test specifically. There is one paper on comparing the Silfverskiöld test to a device to measure ankle dorsiflexion (Molund, M., Husebye, E. E., Nilsen, F., Hellesnes, J., Berdal, G., & Hvaal, K. H. (2018). Validation of a new device for measuring isolated gastrocnemius contracture and evaluation of the reliability of the Silfverskiöld test. Foot & ankle international, 39(8), 960-965). It looked at the intraclass correlation coefficient for inter and intrarater reliability. It was found that the device had an ICC of over 0.85 whereas the Silferskiod test had an inter of 0.23 and intra of 0.791. In a another paper, the test was looked at to see how valid it was to evaluate a spastic equinus foot caused by either the gastroc or soleus being overactive (Picelli, A., DI CENSO, R., Angeli, C., Spina, S., Santamato, A., Baricich, A., ... & Filippetti, M. (2024). Is the Silfverskiöld Test a valid tool for evaluating calf muscles spastic overactivity in patients with stroke? A retrospective observational study. European Journal of Physical and Rehabilitation Medicine, 60(5), 761. ). A nerve block was performed on the tibial motor nerve to the soleus, then gastroc, and then tibialis posterior.
