Recorded on 12/4/2021 60-year-old male with newly diagnosed multiple myeloma with extramedullary plasmacytoma with plasmablastic features Summary: The panel agrees with the diagnosis of extramedullary multiple myeloma with plasmablastic features, rather than plasmablastic lymphoma. Plasmablastic lymphoma tends to have more lymph node involvement, present in oropharyngeal region in the advanced setting, with more radiographic evidence for lymphoma, and be EBV-associated, whereas extramedullary multiple myeloma demonstrates evidence for systemic disease with bone marrow monoclonal plasma cells such as observed in this patient. For a patient with extramedullary multiple myeloma with plasmablastic features, that has been confirmed differentiated from a plasmablastic lymphoma, the treatment approach is the same as for someone with high-risk multiple myeloma. Options include RVD (revlimid, Velcade, dexamethasone), CyBorD (cyclophosphamide, bortezomib, dexamethasone), KRD (carfilzomib, revlimid, dexamethasone), but the panel prefers daratumumab-RVD for inducing remission given the patient is high risk. If transplant eligible, this patient should be considered for autologous stem cell transplant with high dose melphalan, followed by a triple agent maintenance regimen to stabilize the disease control after the transplant.
