Examination demonstrated tortuous and dilated episcleral vessels (Fig A) with near normal best-corrected visual acuity (20/25) and intraocular pressure (21 mmHg). Angiogram confirmed the diagnosis of intraorbital arteriovenous fistulas (Fig B). Two months later, he developed sudden blurring of vision to 20/200. Macular subretinal fluid and dilated choroidal vessels were observed (Fig C, D). We speculate that the hemodynamic abnormality resulted in dysfunction of choriocapillaris and retinal pigment epithelium.
