Hypospadias Masterclass: Session 2

Dr. Snodgrass and Dr. Bush of the Hypospadias Specialty Center present session 2 of their newest series, Hypospadias Masterclass. Last time, we discussed who needs a distal hypospadias repair. This presentation focuses on defining standards for results when surgery is done. It is commonly accepted that results are good after distal hypospadias repair, and many parents are told there is a 95% success rate by the pediatric urologist they consulted with. Certainly there are many publications supporting this claim, but is that actually the outcome parents can realistically expect? Looking at results from the Dutch Hypospadias Study, showing complications in over 800 patients operated at their major centers, we can see that only a few surgeons achieve a 95% success rate, while others had complications in over 40% of their patients. So while success CAN be achieved in 95% of repairs, most surgeons in this study did not meet that standard. Why is there such variation in results for an operation that most pediatric urologists feel comfortable doing and believe they have good outcomes? How can surgeons by certain they are getting the results they quote to parents? How can mentors know they are teaching the operation right to their trainees, since mistakes they teach will hurt the boys these surgeons then encounter?
