Dr. Ebraheim’s educational animated video describes the condition of Bunionette - Tailor's Bunion. Bunionette is also called “Tailor’s Bunion”. It I a prominence usually at the lateral part of the fifth metatarsal head. Pain can result from bursitis. bursitis can happen because of rubbing of the fifth metatarsal head prominence against the shoes. The patient will have a widened forefoot and mya have hyperkeratosis. The condition occurs more in women and could be bilateral. It can be diagnosed by weight bearing AP view radiograph. There are three type of Bunionette (type decides the treatment): Type I: •The fifth metatarsal head is enlarged. •Normal alignment. Type II: •Lateral bowing of the diaphysis of the fifth metatarsal. Type III •Widened four and five intertarsal angle. •More than 8 degrees is pathological. Treatment •Non-operative: has a high success rate. •Anti-inflammatory medication •Orthosis (Wear proper shoes with a wider toe box and pad the prominence area) •Surgery is rarely needed. Type I •Do resection of the lateral part of the fifth metatarsal head. Type II •Do distal metatarsal osteotomy. The fifth metatarsal is cut and the metatarsal head is repositioned. Type III •When the intermetatarsal (IM) angle is more than 12 degrees, do oblique diaphyseal osteotomy. •Avoid proximal osteotomy in the fifth metatarsal due to the possibility of interrupting the important blood supply. •There is also a high rate of nonunion.
