Ovary is the organ through which eggs are produced in a woman. Sometimes the ovary could develop a cyst. Cyst is nothing but a water filled bleb which could grow in size. it could contain water, it could contain blood or any other content but once it grows and becomes heavy, it can tort on its pedicle. The ovary is usually attached to the uterus by a pedicle. The pedicle will contain the blood supplying to the ovaries. When it gets twisted the flowing to the ovary is compromised. Slowly it gets compromised and it starts getting necrosing or it starts dying. So how will the patient usually present with? Usually there is severe pain abdomen, vomiting and dizziness and fainting episodes. So when we examine her, find severe pain abdomen and the ultrasound will be able to pick up that there is an ovarian cyst or the ovary being enlarged with very less blood flow. So how do we manage this? Ovarian torsion is an emergency. We usually do a laparoscopic procedure, that is we go through the keyholes and we go inside and see what is wrong with the ovary. If it is torsion, it can be picked up in the early stages. Then we could detort it. We would release the torsion an then we look for improvement in blood flow if that is not sufficing or if we have diagnosed in very late stage where the entire ovary is necrosed then we might have to remove the ovary itself. But if picked up early we could still save the ovary, we could do cystectomy. Why has it torted? Because of the heaviness or the cyst in the ovary. So that cyst can be removed and once that cyst is removed we can still save the ovary. So it is important to know that torsion of the ovary is an emergency and if attended to at the earliest we could save the ovary.
