Dr. Eric M. Genden, Chair of Otolaryngology – Head and Neck Surgery at the Mount Sinai Health System in NYC discusses the first donor tracheal transplant procedure in detail. He explains that after opening the donor, he and his team defined and dissected the trachea and blood vessels that nourish the graft. Next they decided to proceed and bring in the recipient. They then removed her diseased trachea and proceeded with a microvascular transfer. The graft was placed on ice (cold ischemic time) and the team moved quickly to get the graft into the recipient and revascularized. This required hooking up the arteries to bring blood to the graft and the veins to get the blood out from the graft. They then saw the graft come alive and bleed throughout the entire trachea down to the lungs, indicating it was well vascularized. This was the biggest obstacle historically in previous attempts at tracheal transplants. Having revascularized the graft and completing a successful tracheal transplant at Mount Sinai was an historic event, as it was the first ever worldwide.
