The SHORT Case Of The Day! This patient had symptoms of Maxillary Sinusitis of Endodontic Origin (MSEO). Endodontic treatment was performed and patient's symptoms of tooth pain and sinusitis went away almost immediately. Limited field CBCT imaging has been shown to significantly improve the ability to detect odontogenic sources for sinusitis. In a study by Low, CBCT revealed 34% more lesions than periapical radiography, as well as significantly more expansion of lesions into the maxillary sinus, sinus membrane thickening, and untreated canals. Inflammatory responses of the maxillary sinus to dental infection can present with varied symptoms, clinical progression, and radiographic presentations. Periapical mucositis (PAM), which appears on CT imaging as a mucosal thickening or dome-shaped soft tissue expansion in the floor of the sinus directly adjacent to the infected root apex. A healthy vital pulp will not contribute to any periradicular or odontogenic sinus inflammation. Only those teeth with an infected necrotic pulp or failing endodontic treatment will generate MSEO.
