Pediatric liver transplantation outcome has been much improved in the eighties-nineties, but lack major improvements in terms of surgical technical success and long-term outcome in last 2 decades. Slow progresses, yes, but the serious problems of 1 the persisting pre-transplant mortality and 2 the very early graft and patient loss, have not been challenged by surgeons in the adequate manner. Solutions for rapid improvement, by hands of surgeons, exists since many years. It is time to challenge this situation rather than going forwards with only the new "a la mode" technical developments... Let's first eliminate early graft loss and need for re-transplantations, and the associated preventable deaths...
