So, stay tuned. ▬ 📌 Henoch-Schonlein Purpura (HSP) We will be discussing Henoch-Schonlein Purpura, which is an autoimmune disorder more common in children affecting the joints and resulting in skin manifestations. So, let’s dive in! 🟡 It is more common in children and males are predominantly affected. 🟡 There is also an association with HLA B 34 HLA DR B 1 allele 1. HSP is a feature of the genetically determined auto-inflammatory disease, familial Mediterranean fever (FMF) due to the same genetic associations. ▬ 📌 Henoch-Schonlein Purpura (HSP) Pathogens Pathogens involved in Henoch-Scholein Purpura are: 🔵 Group A beta-hemolytic streptococcus (GAS) has been widely studied and found in 20–50% of patients with acute HSP by serological tests or bacterial cultures 🔵 Bartonella henselae (12 of 18 HSP patient sera were positive) 🔵 Other HSP-associated pathogens have been reported 🔵 Staphylococcus aureus, 🔵 Helicobacter Pylori, 🔵 Hemophilus parainfluenza, 🔵 Coxsackie virus, 🔵 Adenovirus, 🔵 Hepatitis A virus, 🔵 Hepatitis B virus You can learn more about Henoch-Scholein Purpura pathophysiology in the video. ▬ 📌 Henoch-Schonlein Purpura (HSP) Clinical Presentations The mnemonic HSP will help you remember them easily. So, here the: 🟢 H stands for hematuria 🟢 S stands for a small child 🟢 P stands for palpable purpura on the buttocks The last A in purpura Stands for the significance of Ig A-mediated immune response. ▬ 📌 Henoch-Schonlein Purpura (HSP) Lab investigations ⚫️ Platelet count and coagulation studies to look for other causes of bleeding. ⚫️ Laboratory tests to rule out other causes of vasculitis. ⚫️ Evaluation of kidney function by blood pressure check, creatinine level, electrolytes, and urine sample. ⚫️ In some patients, a biopsy may be taken of the skin, kidney, or other tissue. Biopsies in patients with HSP often show high levels of a specific type of immune protein, called immunoglobulin A (IgA). ⚫️ Imaging of the bowels may be performed if abdominal pain is severe. ▬ 📌 Henoch-Schonlein Purpura (HSP) Treatment Though, there is no specific treatment for Henoch-Scholein Purpura, but: 🟢 Joint pain can often be controlled with rest and over-the-counter medications such as acetaminophen, ibuprofen, or naproxen. 🟢 Steroids given by mouth or through an IV infusion are typically given for severe abdominal pain. 🟢 Kidney disease has to be closely monitored.
