Diagnosis requires the clinician to collect and integrate complex, uncertain, information from multiple sources. To do this they use a heuristic cognitive process - a combination of pattern recognition and simplifying assumptions. When this process works well, it allows clinicians to work in fast paced pressured environments, however, when it goes wrong, they result in predictable errors in judgment.
