Read full article: https://link.springer.com/article/10.... This retrospective cohort study analyzed 50 cases of periprosthetic joint infection (PJI) in megaprostheses (MP) from 2010–2022. The overall infection-free survival was 42%, with 14% amputations and 14% PJI-related deaths. Two-stage revision achieved 71% success, multi-stage 57%, DAIR 39%, and single-stage 0% (P = 0.009). DAIR success differed by infection type: 55.6% in acute vs. 15.4% in chronic PJI. Coagulase-negative Staphylococci were the most common pathogens (42%), while Gram-negative infections had 100% failure. The study concludes that two- or multi-stage revision is most effective, while DAIR is limited, especially in chronic cases, and may be considered only if extensive surgery is not feasible.
