Inyección controlada de RTPA subretiniano mediante VFI

Paciente con hemorragia macular subretiniana secundaria a vasculopatía coroidea polipoidea idiopática. Se realiza tratamiento mediante antiVEGF intravítreo y a las 48 horas vitrectomía para inyección subretiniana de RTPA (50ug/0.1ml). La inyección subretiniana se realiza mediante cánula de 41g de Dorc, retráctil y con tubo intermedio que permite no transmitir vibraciones por la manipulación de la jeringa a la punta de la cánula. Conectamos la cánula al sistema de inyección de fluidos de alta densidad del vitreotomo (VFI) para así tener completo control sobre la presión de inyección, y poder hacerlo con el pedal facilitando la maniobra. A los 7 días se observa resolución completa del hematoma, permaneciendo restos hemáticos junto al pólipo. Se continúa con inyección intravítrea de antiVEGF mensual. The subretinal injection is carried out using a 41g Dorc cannula, retractable and with an intermediate tube that allows not to transmit vibrations due to the manipulation of the syringe to the tip of the cannula. We connect the cannula to the vitreotome's high-density fluid injection system (VFI) in order to have complete control over the injection pressure, and to be able to do it with the pedal, facilitating the maneuver. At 7 days, complete resolution of the hematoma was observed, with hematic remains remaining next to the polyp. It is continued with a monthly intravitreal injection of antiVEGF.
