Speaker: Dr. Kranthi Kumar Topic: Neurological complications following neck dissection In this video, Dr. Kranthi talks about: a. Greater Auricular Nerve Injury This sensory nerve, arising from C2 and C3, can be inadvertently damaged during incisions, particularly when raising subplatysmal flaps, leading to sensory loss in the ear and parotid region. Careful depth perception during incision is crucial for preservation. b. Marginal Mandibular Nerve Preservation Injury to this branch of the facial nerve, supplying lip depressors, results in a noticeable asymmetry of the smile and potential drooling. Techniques for identification and meticulous dissection, especially near the angle of the mandible and facial vessels, are vital. c. Hypoglossal and Lingual Nerve Considerations The hypoglossal nerve, supplying tongue movement, is at risk during level 1B and 2 neck dissection, potentially causing speech and swallowing difficulties. The lingual nerve, often near the submandibular gland, can be injured, leading to numbness and taste alteration. d. Spinal Accessory Nerve Management Damage to this motor nerve, supplying the trapezius and sternocleidomastoid muscles, results in shoulder dysfunction and pain. Identification using anatomical landmarks and careful dissection, especially in levels 2B and 5, is essential for preserving shoulder function. e. Phrenic Nerve Vulnerability Injury to this nerve, innervating the diaphragm, though less common, can occur during dissection near the transverse cervical artery or during chyle leak repair. Paralysis of the diaphragm can lead to respiratory complications, especially in patients with pre-existing conditions.
