Small pupil phacoemulsification may be performed without pupillary expansion devices. Certain principles must be adhered to: 1)use intra-cameral pharmacologics to cause pupillary dilation, 2)Stretch pupil if necessary, 3)make capsulorhexsis larger than pupullary margin, 4)phaco within pupillary margin and in center of eye when possible, 5)if difficult removal of cortex use silicone tipped I&A or remove after lens implantation, 6) if having problems with visualization may use iris hooks or pupillary expansion devices.
