Secretory Otitis Media - also called Glue ear is a common condition of the ear. Secretory- related to secretions Otitis - inflammation of the ear Media- middle Though most common in children it is also seen in other age groups. The commonest age in children is from 2-6 years of age. Children typically leave home for the first time at about 2-2 1/2 years of age. By this time their maternal antibodies (through the mother's breast milk) have worn off and they are faced with a barrage of infections from grown ups and other children. Not being used to such a high load of infections from others, children at this age frequently fall sick- specially during the winter months - due to the winter viruses. The nose and throat are closely linked with the ears - infection from the nose and throat travels via the Eustachian Tube into the middle ears. This causes middle ear blockage, infection, or build-up of fluid in the middle ear. Enlarged and infected Adenoids and Tonsils are a frequent source of infection for the ears. DIAGNOSIS 1. History - The diagnosis is usually made on the basis of the typical history followed by 2. Ear examination- typical appearance on Otoscopy of a dull ear drum, retracted or red and hyperaemic. 3. Pure Tone and Impedance Audiometry- to look for hearing loss and middle ear pressure changes 4. X-ray to look for enlarged Adenoids TREATMENT 1. Medical management- with mucolytic, nasal spray, and some exercises 2. Surgical management - In case the blockage persists for long or in the presence of persistent hearing loss surgery may be advised. In this surgery, called Myringotomy with Grommet Insertion, a small incision is made in the eardrum, fluid is extracted from the middle ear and a small tube, called a Grommet is introduced in the ear. This tube stays in place for a few months and then comes out on its own. Grommets may be made from Teflon, Titanium, Steel, or other non-reactive substances.
