It includes two processes. PGD (preimplantation genetic diagnosis) and PGS (preimplantation genetic screening). Patients with advanced maternal age, severely low ovarian reserve, a history of prior miscarriages, a history of prior repeated implantation failures following IVF, a prior history of having a genetically abnormal child, or a family history of genetic disorders are typically those for whom we may suggest a PGT. Additionally, we now recommend PGT occasionally in situations with hereditary cancers, such as some cases of breast cancer. PGT involves removing cells from an embryo—either a day-three embryo or a blastocyst—and attempting to determine the embryo's genetic make-up before inserting the embryo into a woman's uterus. The most common issue found when taking a PGT is mosaicism. When one or two embryonic cells have diverse genetic make-up, this is referred to as "mosaicism." This merely complicates our care. We are unsure about what to do with that embryo, whether to remove it from the woman or to implant it again.
