What does it mean when you have a "non-union" fusion? | Dr. Todd Lanman

First, know what happens when surgeons perform a spinal fusion. When a fusion is performed, the surgeon fuses together two or more of the bones in your spine. We call them vertebrae. These are the bones that allow us to bend and flex our bodies. They also protect the spinal cord that runs the entire length of the spine, from your brain to the base of the hips.  Spinal fusion is major surgery. It requires that the surgeon remove the disc, then install a system of rods and plates to the affected vertebrae. Then we add bone grafts between the vertebrae to strengthen the fusion. If all goes well, in about three months the vertebrae involved will fuse as one bone. However, occasionally, the bone graft doesn’t grow, and the vertebrae do not fuse. That’s when you have a “non-union fusion.” If you have a lot of pain after three months, your surgeon will check the graft to see if it did not join.  In my experience, the only way to detect a non-union is by CAT scan. X-rays are not good enough. We need to see a detailed image of the fusion to see what’s going on with the bone. A CAT scan will slice through the bone and tissue and show us every detail about the health and condition of the fusion.  Dr. Lanman is a board-certified neurosurgery specialist practicing medicine for 25 years with a private practice in Beverly Hills, CA. He specializes in the treatment of spinal disorders and is a leading clinical advocate for artificial disc replacement surgery. He is a professor at the University of California Los Angeles, and a member of the Cedars-Sinai Institute of Spinal Disorders. Part of his constant advocacy for patients stems from his own experience as a patient. His unique insights helped him develop a program called "4D Health Process." His stated goal is to help patients be greater than "better." Dr. Lanman graduated from Northwestern University School of Medicine, Chicago, IL with top honors and is a member of the Alpha Omega Alpha Honor Medical Society. He completed a general surgery internship and neurological surgery residency at UCLA. He is an active member of many professional societies and organizations including the American Association of Neurological Surgeons (AANS), Congress of Neurological Surgeons (CNS), International Society for the Advancement of Spine Surgery (ISASS), North American Spine Society (NASS), and Fellow American College of Surgeons. He is a Diplomate of the American Board of Neurological Surgery, a Fellow of the American College of Surgeons, and a Fellow of the American Association of Neurological Surgeons (FAANS).
