Divide and Conquer Technique for Cataract Extraction. Key points: 1. Groove, crack and rotate. Groove distal and deep. The deeper the groove is, the easier the cataract will be to crack. If cataract does not crack, groove distal and deeper, until red reflex is visualized. To rotate, need good hydrodissection. If hydrodissection is not achieved initially, can hydrodissect after central groove and crack is made. With a crack in the middle, upon more hydrodissection, do not need to be afraid of blowing out posterior capsule because BSS will propagate through the central crack. 2. Wound in this case is enlarged because patient is a high myope and 3 piece IOL was injected, which cannot fit through a 2.2 mm incision. Enlarge incision from inside out.
