Overview Meniscal ossicle is a rare entity, often resulting from posttraumatic heterotopic ossification or small occult bony avulsion fractures. Commonly observed in individuals with knee pain and a history of trauma. Associated with a high likelihood of meniscal tear, articular cartilage loss, ACL injury, or meniscal extrusion. Trauma History: Present in 60% of patients. Knee Pain: Reported in 87% of cases. 2. Localization Medial Meniscus: Observed in 88% of cases. Lateral Meniscus: Found in 12% of cases. Posterior Horn or Root: Present in 93% of cases. Posterior Horn: 43%. Posterior Root: 50%. Anterior Horn or Root: 4%. Midportion: 3%. 3. Associated Pathologies Meniscal Tear: Found in 76% of patients. Articular Cartilage Loss: Present in 61% of cases. 4. MRI Features Well-defined focal lesion within the meniscus. Isointense to adjacent bone marrow on all pulse sequences. Surrounded by a thin hypointense rim, representing cortical bone. 5. Differential Diagnosis Can mimic a loose body on radiographs. Increased signal intensity on MRI can be mistaken for a meniscal tear. Review radiographs to prevent false-positive diagnoses of meniscal tears. 6. Management Symptoms arise from mass effect or associated tears. Treated effectively with arthroscopic resection.
