The surgery was perfect; our student placed the transnasal and zygomatic implants exactly according to the virtually osteotomized prototype, under our guidance However, despite good anchorage, we had issues with the anterior implants. We plan to remove them and install a second zygoma (called double zygoma in Brazil, referring to the treated hemi-arch; in the literature, the term quad zygoma is more commonly used). On one hand, we are saddened by the failure of a procedure in our patients; however, this does not represent the failure of the treatment. The patient will undergo reoperation and receive immediate loading. We have here an excellent opportunity for students to learn that even when we do everything right, there may be cases of failure, but because we made a good initial plan and an excellent surgical procedure, we will be able to overcome this occurrence. ------------- 🇧🇷planejando aqui as cirurgias da próxima edição do nosso curso internacional e me deparei esse interessante caso no qual tivemos um insucesso. A cirurgia foi perfeita, o aluno instalou os implantes transnasais e os zigomáticos exatamento de acordo com o protótipo virtualmente osteotomizado, sob nossa orientação porém, apesar da boa ancoragem, tivemos problemas nos implantes anteriores. Planejamos removê-los e instalaremos um segundo zigoma (no Brasil chamados de duplo zigoma, nos referindo ao hemi-arco tratado; na literatura, o termo quad zygoma é mais empregado). Por um lado, ficamos tristes pelo insucesso de um procedimento em nossos pacientes, porém isso não representa o insucesso do tratamento. O paciente será reoperado e terá carga imediata. Temos aqui uma ótima oportunidade para os alunos aprenderem que, mesmo quando fazemos tudo certo, podem haver casos de insucesso, mas por termos feito um bom planejamento inicial e um excelente procedimento cirúrgico, conseguiremos contarnar essa ocorrência. #zygomaticimplant #transnasal #pterygoid #allon4 #zygomaticimplant #implantezigomatico #implanteszigomaticos
