Recent advances in endoscopic endonasal approaches have expanded their boundaries to adequately reach many skull base lesions. However, treatment of vascular pathologies by this method has not yet become widespread, as in the vast majority of cases traditional transcranial approaches provide the most appropriate microsurgical treatment. The main indication for endoscopic endonasal vascular surgeries are deep-seated lesions, especially posterior circulation aneurysms. It is claimed that the endoscopic endonasal approach provides better control over these aneurysms, with less intraoperative rupture and less brain retraction. Currently, there is no consensus on the usefulness of the endoscopic endonasal approach in intracranial vascular lesions. Professor Paul A.
