Left Femoral subintimal angioplasty for chronic total occlusion (Flush Occlusion)

Image shows chronic total occlusion of superficial femoral artery. 1. Attempt to raise intimal flap using stiff end of a standard guiode wire. 2.Once wire is secured, a sheath is introduced. 3. Using a hydrophilic wire (Terumo) supported by 4F catheter such as Cobra (Cordis), a loop is created with flopy end of the wire. 4.The loop is advanced through the new channel, making sure the catheter is following the loop at all time. 5. Keep the size of the loop curve as small as possible to avoid arterial rupture. 6.Once you reach the distal end of the occlusion the wire loop will set it self free into the true patent lumen. This can be appreciated by immediate sense of the wire giving way into the artery lumen without further increase in the loop size. 7. Exchange the catheter with a suitable size balloon for completion of angioplst by dilating the new channel.
