When a surgeon is performing a breast augmentation, there are two options for where to place the implant; under the muscle or over the muscle. The more common placement is under the muscle. When a surgeon chooses to place the implant above the muscle, they have to be very careful to only create a large enough pocket to contain the implant. As Dr. Richard Brown explains, there is extreme caution in both scenarios to not release the muscle along the sternum area. If the pocket is created too large and the pectoralis muscle is released from the sternum bone, the implant is now free to move across the midline of the chest. Put simply, your breasts are free to move to the wrong side. That is symmastia and it can be a very large complication. Even in correction, it's possible to re-attach skin tissue but we often don't have the ability to re-attach the muscle to the sternum bone. So how do we correct symmastia? The first thing that a surgeon will need to do is go in and remove the current implants to be able to look inside to see where the inside skin has been elevated from the sternal bone. The tissue will be sutured either directly to the chest wall or with a piece of mesh. This will help to create new pockets to hold the breast implants in place.
