In this video, I demonstrate my technique of E-DMEK or Endoilluminator assisted DMEK. Here, the endoilluminator is used to enhance visualization and 3-dimensional perception of the DMEK graft. This helps the surgeon greatly to comprehend more easily graft position, orientation, morphology and dynamics, all of which leads to easier and faster surgery. It avoids unnecessary maneuvers that might otherwise be required to analyze graft position and thereby decreases potential graft damage. It makes determining graft orientation with respect to endothelial versus Descemetic side a non-touch technique. Light from the endoilluminator is shown obliquely while the microscope light is switched off. This light bouncing off the folds and edges of the DMEK graft allows very good 3-dimensional perception even in a hazy, edematous cornea.
