Speaker: Dr. Vijay Pillai Topic: Sialocele and Parotid Fistula In this video, Dr. Vijay talks about: a. What is Sialocele and Parotid Fistula? A sialocele is a collection of saliva in the subcutaneous tissue due to salivary gland or duct disruption, while a parotid fistula is an abnormal tract draining saliva externally from the gland or duct to the skin. b. Anatomy of the parotid duct The parotid duct typically runs from the tragus to the philtrum, closely related to the facial nerve branches, crossing the masseter, piercing the buccinator, and opening into the oral cavity opposite the second molar. c. Related etiology and risk factors Sialoceles and parotid fistulas commonly arise from surgical procedures (especially parotidectomy and oral cancer surgery), trauma, ductal injury or obstruction, infection, and neoplasms, with inferior and middle parotid resections carrying higher risk. d. Diagnosis and Imaging Modalities Diagnosis of sialocele often involves clinical history and ultrasound, while parotid fistulas may require sialography or MRI to delineate the drainage tract. e. Interventional Treatments: Botox, Surgery, and Tympanic Neurectomy Management options range from conservative measures like pressure dressings and antisialogogues to interventional approaches including Botox injections into the parotid gland, surgical repair or stenting (rarely used), and tympanic neurectomy to reduce salivary flow.
