Avascular necrosis (AVN), also known as osteonecrosis, progresses through stages that can vary in duration depending on factors like the cause, location, and individual patient health. Stage 2 AVN is characterized by early changes in bone structure detectable by MRI but often not visible on X-rays. During this stage, patients may begin experiencing pain and limited joint function. The progression from stage 2 to stage 3 or 4 can be relatively rapid or slow. Stage 3 involves the collapse of the bone's surface, leading to more severe pain and functional impairment. In stage 4, there is significant joint damage and arthritis. The speed of this progression can range from a few months to several years. Factors influencing the progression rate include the underlying cause of AVN (such as corticosteroid use, alcohol consumption, or trauma), the effectiveness of early treatment interventions (like medications, reduced weight-bearing, or surgical procedures), and the patient's overall health and compliance with treatment recommendations. Regular monitoring through imaging and clinical evaluations is crucial to manage and potentially slow the progression of AVN, aiming to preserve joint function and delay the need for more invasive treatments like joint replacement.
