Inverted inner limiting membrane flap technique for large macular holes

Bridging the gap in large macular hole using the inverted inner limiting membrane (ILM) flap technique. 23 gauge pars plana vitrectomy was performed. The ILM was stained using the ICG dye. Using ILM forceps, the inverted ILM flap was created and tucked into the macular hole. Careful fluid air exchange was performed to make sure the inverted flap remained stable in position, followed by infusion of perfluoropropane gas (C3F8 14%). Face down positioning for 3 days was advised. This large macular hole (basal diameter of 1375 microns, axial length corrected basal diameter of 1424.3 microns) was successfully closed, and the vision improved from 20/400 to 20/100 within 2 months after surgery in this phakic patient.
