26)hysteroscopic myomectomy in prev 2 lscs by tissue retrieval system

Previous 2LSCS with large sub mucous fibroid with menorrhagia at 49 years of age refused TLH. After proper work up she was counselled for diagnostic Hysteroscopy followed by hysteroscopic morcellation under anaesthesia. On mapping the fibroid, it arose from post left lateral wall with a large base , vascular. So intra cervical diluted vasopressin 10cc at 4°clock and 8°clock was injected to decrease the vascularity and then truclear elite tissue retrieval system was inserted without any cervical dilatation. Since the incisor blade skid of the surface the fibroid was pierced at its soft part and morcellation carried out from within out, that is from substance of fibroid to its capsule , there by reducing the volume of the fibroid, saving time and also decreasing fluid absorption
