It is difficult to take a full history due to a language barrier What is the differential for the X-Ray findings? ———————— EXTRAMEDULLARY HAEMATOPOESIS 👨🏽💻This is a tricky X-Ray to decipher and comes down to understanding two concepts: 1. The hilum overlay sign - if the hilar vessels overlie a mass, the lesion likely does not arise from the middle mediastinum meaning anterior and posterior mediastinal lesions are more likely 2. Looking for abnormality within the posterior mediastinum: look at the vertebrae, azygo-oesophageal recess, descending aorta and paraspinal lines 👨🏽💻Here the left paraspinal line is abnormal pointing us towards the posterior mediastinum 👨🏽💻Once we know there is a history of thalassemia the diagnosis becomes more straight forward. CT showed the characteristic fat containing paravertebral masses as well as splenomegaly 👨🏽💻Biopsy of these lesions is very high risk given potential haemorrhage so remembering the differential when there is an underlying predisposing condition is important FIND THIS USEFUL?
