Mouth: Inspect the lips for color, moisture, cracking, or lesions. Inspect the teeth: Teeth should be white, straight, evenly spaced, and clean. Inspect the gums: they should be pink with no swelling or bleeding. Inspect the tongue for color, surface, characteristics, and moisture. Ask the person to touch the tongue to the roof and sides of the mouth. Hold the cheek open with a tongue depressor to check the buccal mucosa for color, nodules, or lesions. It should be pink, smooth, and moist. Shine your penlight to inspect the hard and soft palate. Observe the uvula. Ask the patient to say "ahh" and note the soft palate and uvula rise. This tests the function of cranial nerve X (vagus). Throat: Inspect the tonsils. They are graded as follows: 1+: Visible (Normal) 2+: Halfway between tonsillar pillars and uvula 3+: Touching the uvula 4+: Touching each other There should be no exudate or swelling. Touch the posterior wall with the tongue depressor to cause gag reflex. This tests cranial nerve IX (glossopharyngeal) and nerve X (vagus). Note any halitosis.
