Hydrosalpinx is a chronic inflammatory condition of the tubes which is commonly caused by organisms like chlamydial organism or the tubercular infection where there is a damage and there is an exudation of the fluid into the cavity into the lumen of the tubes and there is distension of the tubes so since it is an inflammatory reaction, people ask whether it can be cured with medicines. If it is an acute condition it can be treated with a prolonged course of antibiotics but this is a chronic condition where the tubal damage has happened to a certain extent. There is not much of a role in the present course of antibiotics in this condition and it had to be treated in a manner where it improves the fertility in the future for these patients so to improve the fertility, we have to disconnect the damaged tubes for the ureters. That is prevent the toxic fluid from coming to the uterus when the implantation is happening. So the only way to do it is the ligation of the tubes and causing fenestration of the distal part so that the fluid release does not happening to the peritoneal cavity and the distension does not happen because any distended organ is a cause of pain in the pelvic area. There are a lot of patients with hydrosalpinx with pain as well. So it prevents the pain and prevents the proximal ligation of the tubes, prevents the fluid comes back into the uterus and thus increases the pregnancy rates. These surgical procedures are done most often through a laparoscopic approach as it is least invasive and it involves giving general anesthesia and the tubes are disconnected and if there is unilateral hydrosalpinges, there is a hydrosalpinx of only one tube and you gave to treat only the affected tube and if the other tube is healthy you can leave the other tube intact and ask the couple to try naturally for a couple of months for the pregnancy to happen if it is actually happening or not.
