Genital Tuberculosis 🦠 OBG for USMLE Step 2 CK | Pathogenesis, Diagnosis & Infertility In this high-yield lecture, we cover genital tuberculosis (TB), an often under-recognized cause of infertility and chronic pelvic disease. While more common in developing countries, it is an important condition to understand for USMLE Step 2 CK, particularly in patients presenting with infertility, menstrual irregularities, or pelvic pain. We begin with the pathogenesis, explaining how Mycobacterium tuberculosis spreads hematogenously or via lymphatics from a primary pulmonary or extrapulmonary focus to the genital tract. The fallopian tubes are most commonly affected, followed by the endometrium, ovaries, cervix, and rarely the vagina or vulva. Clinical features include infertility (most common presentation), menstrual disturbances (amenorrhea, oligomenorrhea, menorrhagia), chronic pelvic pain, and adnexal masses. Many patients remain asymptomatic until evaluated for infertility. Diagnosis involves a combination of: Imaging: hysterosalpingography (HSG) showing rigid, beaded tubes; ultrasound/MRI for adnexal disease. Endometrial biopsy: granulomas and positive acid-fast bacilli (AFB). Microbiological confirmation: culture, PCR, GeneXpert/NAAT for TB. Management is primarily medical, using standard anti-tubercular therapy (isoniazid, rifampicin, pyrazinamide, ethambutol). Surgical intervention may be required for tubo-ovarian masses or severe pelvic adhesions, but fertility outcomes are often poor due to irreversible tubal damage.
