This patient elected to have bilateral cataract surgery with multifocal / trifocal IOLs but then did not like the quality of vision. While these trifocal IOLs can give patients a wide range of vision without glasses, they accomplish that by splitting / spreading light. The number of incoming photons is the same, so if you put all the light energy at one focal point (monofocal IOL) then the image quality is great. If you split/spread the light energy over multiple focal points, the image quality must be less. You can't cheat physics. Our guest surgeon does a great job of bilateral (sequential) IOL exchange and replaces the trifocal IOLs with monofocal IOLs. The patient then achieved better image quality but then, of course, became more reliant on glasses.
