This video shows Hematometra in 25 years old unmarried lady. Hematometra or Hemometra is a medical condition where collection or retention of blood is seen in the uterus. It is most commonly caused by an imperforate hymen or a transverse vaginal septum. Hematocolpos is a medical condition in which the vagina fills with menstrual blood. It is most commonly caused by menstruation with an imperforate hymen. The diagnosis of hematometra is generally suspected by the history of amenorrhea and cyclic abdominal pain. The diagnosis is usually confirmed by vaginal ultrasound or probing the cervix with a narrow metal dilator, with the release of dark brownish-black blood from the endocervical canal. Enlarged uterus with a distended endometrial cavity containing fluid and heterogeneously echogenic material (representing blood products). An ultrasound examination may be helpful in identifying an enlarged uterus and differentiating that from a normal pregnancy. Ultrasound changes that indicate pyometra include increased uterine size, thickened uterine walls, and fluid accumulation within the uterus. Hematometra typically presents as cyclic, cramping pain in the midline of the pelvis or lower abdomen. Patients may also report urinary frequency and urinary retention. It is a rare condition that is most commonly associated with congenital anomalies or prior surgical procedures causing obstruction of the genitourinary outflow tract. Hematometra is a rare but potential cause of pelvic pain in women, especially those who may have undergone cervical procedures leading to scarring. If left untreated, the infection may spread to the abdominal cavity (leading to peritonitis, the inflammation of the membrane lining the inside of the abdominal cavity) or to the bloodstream (causing sepsis). Background. Hematometra or Hemometra is a medical condition involving the collection or retention of blood in the uterus. It is most commonly caused by congenital abnormalities of the cervix or uterus. Less commonly it can be acquired due to processes that cause obstruction of the cervical canal. Reproductive tract (Mullerian) anomalies causing an accumulation of menstrual blood in the vagina, uterus, and fallopian tubes represent an unusual extraspinal cause of low-back pain. The accumulation of blood in these cavities is termed hematocolpos, hematometra, and hematosalpinx. Pregnancy is a common cause. Polyps or fibroids (small and large growths) in the uterus can also cause bleeding. Rarely, a thyroid problem, infection of the cervix, or cancer of the uterus can cause abnormal uterine bleeding. In most women, abnormal uterine bleeding is caused by a hormone imbalance. The surgical procedure of hymenotomy and evacuation of hematocolpos is performed in an outpatient setting. The patient and family should be instructed to expect continued drainage of dark, thick, old blood for several days to a week after the procedure. Mild cramping may occur as the hematometra resolves and evacuates. We report an unusual case of hematometra with endometriosis secondary to cervical stenosis. This is a rare and important case report due to the complexity of diagnosis as cervical stenosis was not presented as primary amenorrhoea as its usual presentation.
