Obstetrical forceps are a surgical instrument that is used to assist the obstetrician in delivering the child through the birth canal. Forceps look like a long pair of spoons. Doctors and nurses have been using forceps to deliver babies for 500 years. There are many different types – Simpson, Kielland, Wrigley’s, Pipers, Elliot – that have different uses in different situations. But they are largely the same and there has been minimal innovation since they were invented. Forceps look like tongs (we have our own forceps as you will see in the video) that fit the baby's head. The idea is if the baby needs a little bit of help, the forceps pull the baby along just enough for extraction.  Of course, you are pulling a newborn baby's head - the OB needs to be gentle. Birth injuries can result when the obstetrician misuses the forceps. Our birth injury lawyers see two common types of cases. One recurring problem we see is when the obstetrician incorrectly positions the forceps. Similarly, in the heat of the moment, many OBs forget that forceps delivery is counter-indicated until the fetal head is low enough to ensure safe delivery. Brain bleeds can be the result of the failure to keep the forceps off the child’s brain and skull. This can lead to skull fractures, intracranial hemorrhage, and lifelong injury. The second problem is that, often in a panic, the doctor will rotate the head or apply excessive traction to accomplish the delivery. Each of these maneuvers is associated with increased forceps related trauma that can also cause head and brain injuries. One of the big problems today is that younger obstetricians are focused on Caesarean section or Ventouse (vacuum extractor) as the path to dealing with a problem delivery. But if the baby has a non-reassuring heart tracing, there are times when the delivery is too far gone for a C-section (for example, in some cases when the vertex is well below the ischial spines). So when the forceps are called for, they have little experience using them. All the evidence tells us that using forceps is a skill that gets better with practice. Today, the need to be able to use forceps is arguably greater than ever because women are giving birth later which can lead to more complications in the delivery process. Many believe forceps should be banned, mostly women who have had a bad experience with them. The argument is that since the use of forceps during childbirth is associated with higher rates of birth injuries, shouldn't we just take away the forceps? We are lawyers, not doctors so take our spin on this with a grain of salt. We would agree that forceps should only be used when absolutely necessary. But it is a weapon obstetricians should have available to them. They just need to be well-trained and use forceps only when appropriate.
