The os peroneum is an extra bone in the foot that is found within one of the peroneal tendons off of the cuboid bone and the base of the 5th metatarsal bone on the outside of the foot. These are pretty common being found in over 20% of people. If you have an os peroneum, you will be more prone to tendonitis and pain of the peroneus longus tendon on the outside of the foot. When treating people who have pain associated with os peroneum we have a goal to reduce tension on the peroneal tendon as will usually eliminate pain associated with os peroneum. We will often prescribe orthotics or arch supports to reduce tension on the peroneal tendons and in many cases this is all that is needed for treatment. In some cases we need to immobilize the foot in a walking boot or ankle brace for several weeks to allow healing. We often use orthotics for treating pain associated with os peroneum and other accessory bones of the feet. Orthotics should be designed to reduce tension on the peroneus longus tendon and also act to more force from the outside of the foot to the inside arch.
