Preparation includes patient positioning, safety checklists, IV access, antibiotics, and tubes. Access is often via a midline abdominal incision. The surgeon explores, retracts intestines, and controls the aorta above the problem area. A synthetic Y-shaped graft is sewn from the healthy aorta down to the femoral or iliac arteries, creating a new blood path. Blood loss is managed with a cell saver. Constant monitoring is crucial, managing challenges like hypothermia and acidosis. After checking for bleeding, the incision is closed in layers. The patient goes to PACU with a handover checklist. Long-term management is vital. Key steps are managing risk factors: permanent smoking cessation, strict diabetes control, and blood pressure management. Antithrombotic medications like aspirin or clopidogrel are essential for graft function and preventing clots. Structured exercise is beneficial. Critical warning signs, signalling acute ischemia, are the 6 Ps: Pain, Pallor, Pulselessness, Paresthesia, Paralysis, and Poikilothermia. These require immediate medical attention. The surgery is one step in a lifelong journey of care.
