Difficult airway sim with and without an intubation box from AOD

The “intubation” or “aerosol” box was introduced as a presumptive means for protecting health care workers during the aerosolizing procedure of tracheal intubation. This is of special concern when caring for patients with Covid19, and when personal protective equipment is in short supply. Social media posts and letters to the editor expound on the virtues of the “box” with seeming no attention given to the potential downsides. There are myriad ways that the box or similar constructs could do more harm than good, and we won’t know these until controlled studies are undertaken. During simulation one such concern became apparent - the ability or willingness of an assistant to help the intubating personnel during an unanticipated difficult intubation. This is not a “controlled trial” but we tried not to introduce bias into this one simulation event.
