Facial trauma is associated with head injury. So the basic thing is to rule out the head injury. It is basically when you are in an emergency department, we do a CT scan to rule out the head injury. Once we have made sure that there is no intracranial injury, then we address the facial trauma that is associated with it. There are two broad categories of facial trauma. One is the soft tissue trauma, where you have a cut on the skin or a cut in the soft tissue, that is the muscles associated with the facial structure and the second is basically the skeleton that is the injury involving the lower jaw, the upper jaw, sometimes it could be the nasal bone or cheek bone or bone surrounding the eye socket and the forehead bone and this is what the maxillofacial surgeons do. So we address both the hard tissue and the soft tissue trauma. If it is an isolated soft tissue trauma, then we basically debride the area. The basic thing in facial laceration is we debride or we take out the road girth or the road dirt that is there we have to take it out totally or else we get tattooing or we have scarring, which is produced is very bad so when the area is nicely cleaned up we us e fine sutures , that is thinner than hair, that is 5-0,6-0. Unfortunately when we have facial cut we have a scar but the scar can be minimized to a significant extent by a competent surgeon. When we have a hard tissue trauma the bone has to be fixed in its normal position and in its jaw too it has to be fixed in its normal position, and we need to hold it with tiny plates and screws which will allow the jaw to function rapidly. By and large the trauma can be addressed from inside the mouth itself. So you don’t have a scar and we don’t have to make a cut outside and if you have an existing laceration, we usually go through that itself or if you make a cut outside and go through that and we fix it in position. So that is how we manage hard tissue and soft tissue trauma, this cause small discomfort to the patient and patient can get back to normal very rapidly and I remember when we were training, we use to wire the jaws for 6 to 8 weeks and these days our patients go back to normal lifestyle within 24 hours time. They can function normally, they can eat normally, eat and speak normally.
