Class II Amalgam Cavity Preparation – Premolar Welcome to Dr.Dentique-hub 1️⃣ Outline Form • Include all carious pits and fissures on the occlusal and proximal surfaces. • Extend the occlusal portion into the central groove and fissures, keeping 1/4 of intercuspal width. • Extend proximally until you just break contact with the adjacent tooth. • The gingival margin should be 0.5 mm below the contact area and on sound tooth structure. ⸻ 2️⃣ Resistance and Retention Form • Pulpal floor: Flat and perpendicular to the long axis of the tooth. • Axial wall: Slightly convex, following the contour of the tooth. • Buccal and lingual walls: Slightly convergent occlusally (for retention). • Gingival floor: Flat and at right angles to the long axis. • Cavosurface margin: At 90° — no beveling (since amalgam needs bulk for strength). • Depth: • Occlusal depth – about 1.5–2 mm (just into dentin). • Axial depth – around 0.2–0.5 mm inside the DEJ. ⸻ 3️⃣ Convenience Form • Ensure enough access for instrument and amalgam condensation. • Maintain 1 mm clearance with the adjacent tooth at the proximal side. ⸻ 4️⃣ Removal of Carious Dentin • Excavate all infected dentin using spoon excavator or slow-speed bur. • Maintain as much sound structure as possible. ⸻ 5️⃣ Finishing of Enamel Walls • Smoothen all enamel margins. • Ensure unsupported enamel rods are removed. • Gingival margin should be clean and strong, with a butt joint finish. ⸻ 6️⃣ Cavity Cleansing • Rinse and dry the cavity thoroughly. • Avoid desiccating dentin. ⸻ 7️⃣ Lining and Base (if needed) • If the cavity is deep, apply a calcium hydroxide liner on the pulpal or axial wall. • Optionally cover with zinc phosphate or glass ionomer base. ⸻ 8️⃣ Restoration • Insert amalgam in increments and condense firmly. • Carve to reproduce anatomy and maintain marginal ridge height. • Burnish and polish after 24 hours. ⸻ 💡 Key Points • No bevel for amalgam (reduces strength). • 90° cavosurface angle for bulk retention. • Occlusal convergence provides mechanical retention. • Flat pulpal and gingival floors give resistance form. • Preserve the marginal ridge thickness (≥1.6 mm).
