It can happen either because of any sexual contact or any infections or any instrumentation into the urethra by catheter or any endoscopic surgeries. So patient will develop scaring of the urethra, it gets narrowed. So how to diagnose? The patient will have like narrow stream of urine. It comes like a small stream and patient has to strain to pass urine sometimes if you neglect it gets into blockage of urethra leading to pain in the lower abdomen and back pressure on the kidneys. How to diagnose ? One is by clinical examination and history wise you can make out. Second thing we make a ultrasound examination wherein the urine remains in the bladder without empting completely we call it as residual urine. Another thing second we call it as ascending urethrogram are MCU and RGU is a x-ray wherein we inject medicine into the urethra that is urinary PA and take x-rays. So that gives where is the stricture what is the narrowness. How much long is the strictur? Then how we treat it? Suppose if it emergency temporarily will divert the urine suppose patient is having severe pain because of blockage of urine temporarily. We derive the urine either from urethra or from the suprapubically. Then we follow do it like endoscopicaly, I told. Non invasive or minimal procedure. By using Cystoscope from inside only we cut open the that scar tissue. Outside there will not be any wounds from inside only we manage still the patient neglects and it becomes extensive stricture all along. Then we have to do what we call Urethroplasty wherein we take some other tissue from other parts of the body and make a new urethra, that is new urinary passage.
